It is an opportunity to build relationships, advocate for seniors, connect people to life-changing resources, and create a thriving community where residents feel valued and supported.If you are compassionate, organized, and passionate about helping others, we would love to meet you.What You'll DoAs the Service Coordinator, you will serve as a trusted advocate and resource for residents living in St. Joseph Place HUD 202 Senior Housing community.Key responsibilities include:Conduct assessments to identify residents' social, health, and support needs.Provide case management, resource referrals, and service coordination.Connect residents with community services such as transportation, meal programs, counseling, wellness programs, legal resources, personal assistance, and healthcare services.Build and maintain strong relationships with local agencies, service providers, and community partners.Advocate for residents and help them navigate applications, services, and available benefits.Develop individualized service plans and monitor ongoing support services.Help residents strengthen relationships with family, friends, neighbors, and community resources.Coordinate educational programs and training that support successful aging in place.Recruit and develop volunteer opportunities that enhance resident engagement and well-being.Partner with staff to promote a resident-centered environment that values dignity, independence, and quality of life.What Makes You a Great FitWe're looking for someone who:Genuinely enjoys working with older adults.Has strong communication, relationship-building, and problem-solving skills.Is highly organized and able to manage multiple priorities.Demonstrates empathy, professionalism, and sound judgment.Enjoys collaborating with community partners and interdisciplinary teams.Is passionate about helping people access the resources they need to live independently.QualificationsRequiredBachelor's degree in Social Work or a related field ORMinimum of five (5) years of experience in social services.PreferredAt least one (1) year of experience working with older adults or in a geriatric setting.Why Join St. Joseph Place?At St. Joseph Place, you'll become part of a mission-driven organization dedicated to enhancing the lives of older adults.
