The role focuses on ensuring access to necessary medical, social, and community-based services to support independence, inclusion, and overall well-being. Additionally, provides care management services to adults diagnosed with serious mental illness and intellectual and/or developmental disabilities.-### **Key Responsibilities*** Assist with assessments and person-centered service plans* Coordinate and link individuals to internal and external services* Monitor service delivery and ensure plans are implemented as written* Document services, progress notes, and required reports* Conduct periodic follow-up and reassessments* Advocate for individuals and support access to resources* Collaborate with families, providers, and community partners* Support Direct Support Staff and interdisciplinary teams* Maintain compliance with MDHHS, Medicaid, and agency standards* Participate in required training and team activities-### **Additional Responsibilities*** Maintain professionalism in all interactions* Support onboarding and training of new staff as needed* Travel within the service area and statewide as required* Perform other duties as assigned-### **Qualifications*** High school diploma or associate degree* 1 year of experience working with individuals with serious mental illness and/or intellectual and/or developmental disabilities.* Strong organization, communication, and coordination skills-### **Work Environment & Physical Requirements*** Combination of desk work and community-based activity* Frequent standing, walking, and computer use* Ability to lift up to 25 pounds* Visual ability for computer and document review-### **Equal Opportunity Statement**Wellvance Community Mental Health Authority is an equal opportunity employer and does not discriminate based on any legally protected status.### Education details**Acknowledgment and Certification**I certify that all information provided in this application is true, complete, and accurate to the best of my knowledge.
