Schedule: Evening Hours, Flexible Minimum Job Specifications Education – High School Diploma/GED. Licensure/Certification – Job related license/credential (LCSW, LPC, LMFT, MFT, LMSW, LAPC, MAC, CACII, CADC, CAC‑I) or license/credential eligible within 6 months. Experience – Minimum two (2) years of human services, social work and /or clinical experience with addictive diseases population. Job Summary The Counselor provides residential program participants with on‑site clinical services including group training, group counseling, individual counseling, family training, and family counseling, effectively utilizing evidence‑based practices to meet the needs of each individual’s individualized service plan. Seeking to contribute to the success of the program, as well as the organization as a whole, the Counselor successfully carries out daily tasks including facilitation and documentation of group sessions, individual sessions, development of collaborative, individualized service plans, completion of assessments, crisis intervention, and evaluation of participants’ progress, through acquired knowledge, skills, and abilities. Major Duties and Responsibilities Provides individual, group, and family training and counseling including but not limited to psycho‑education, didactic, process, and skills groups, utilizing evidence‑based interventions and practices. Completes daily documentation of group sessions and weekly individual sessions in electronic medical record (EMR) system in accordance with same‑day documentation policy. Completes comprehensive assessments/reassessments with persons served including but not limited to intake screening assessments, bio‑psycho‑social assessments, drug and alcohol evaluations, and vocational skills assessments. Develops, reviews, and updates individualized service plans (ISP) on an ongoing basis, promoting individuals’ self‑articulation of personal goals. Facilitates orientation of newly admitted residents and effective discharge planning, ensuring appropriate documentation has been completed within required time frames. Completes and submits deliverables including but not limited to progress letters and reports in a timely manner, as directed. Attends and participates in case staffings, treatment team, staff meetings, organization meetings, provider meetings, and community events, as directed by direct supervisor. Provides consultation to referral sources, community stakeholders, and members of individuals’ support system to support individuals in their recovery process. Participates in monthly on‑call rotation to provide support to persons served outside of scheduled business hours. Works cooperatively with the continuum of care to ensure coordination of services including collaborating with a multi‑disciplinary team, communicating with criminal justice system, completing referrals to internal and external providers, and court appearances, when appropriate. Provides services consisting of providing environmental support and care coordination considered essential to assist individuals with improving his/her functioning, gaining access to necessary services, and creating an environment that promotes recovery as identified in his/her individual service plan (ISP). Implement interventions such as developing natural supports to promote community integration, identifying service needs, referring and linking to services and resources identified through the service planning process. Coordinate services identified on the ISP to maximize service integration and minimize service gaps, ensuring continued adequacy of the ISP to meet his/her ongoing and changing needs. Research and link individuals with community resources including food security, furniture, clothing, identification, medical care, health insurance, prescription assistance, mental health services, social services, legal resources, self‑help/support groups, transportation resources, vocational resources, and housing resources. Engages in ongoing quality improvement tasks including self‑auditing of electronic medical records, implementation of corrective action plans, and measuring key performance indicators necessary for efficient and effective operation of organization. Strives to deliver services in accordance with accreditation standards, organizational policies/procedures, and contractual requirements. Promotes culture of recovery-oriented care by facilitating therapeutic milieu, maintaining professional relationships with colleagues and community stakeholders, and conducting self in accordance with organization and licensing body’s ethical standards. Intervenes and manages acute crisis situations as needed. Completes in‑person and online training to encourage professional development, maintain certifications/licensure, and meet organization, accreditation, and regulatory standards, as scheduled. Knowledge of addictive diseases and/or mental health disorders and their impact on an individual’s functioning. Knowledge of multiple therapeutic modalities and evidence‑based interventions. Excellent verbal and written communication skills. Exceptional time management skills and attention to detail. Familiar with computer hardware and software including Microsoft Office products (Word, Excel, PowerPoint, Outlook), e‑mail, fax, and scanning. Ability to identify and link individuals with local community resources to aid in their recovery process. Ability to manage and prioritize multiple assignments. Ability to remain professional when dealing with internal and external stakeholders. Ability to maintain accuracy and attention to detail when completing multiple assignments. Ability to work independently and carry out a variety of critical and time‑sensitive projects without detailed instructions. Ability to work with diverse individuals to collect the necessary information. Ability to maintain the confidentiality of sensitive and confidential information obtained through the course of completing assignments. Ability to be flexible in adapting to changes in priorities, assignments, and other interruptions, which may impact pre‑established timelines and courses of action for completing projects and assignments. Ability to establish and maintain cooperative working relationships with management, staff, and internal and external stakeholders.
