Our services focus on supporting seniors and adults in their homes through state-funded programs, working closely with ASAP case managers to ensure safe, compliant, and high-quality care. This position is centered around ASAP consumer care, requiring strong knowledge of community-based services, compliance, and supervisory oversight. About the Role We are seeking an experienced RN or LPN for a full-time, field-based role. This position requires daily travel to client homes to complete supervisory visits, assessments, and ongoing care management. You will be working independently in the field while collaborating closely with case managers, families, and our internal team.
