Role Summary

As a Nursing Specialist , you will serve as a trusted clinical expert, educator, and advocate. You will deliver comprehensive clinical guidance, hands-on training, and proactive support to families, natural supports, and team members caring for individuals with complex developmental and medical needs. In this role, you bring whole-person clinical excellence to help those we serve experience health, safety, and long-term independence.

What You Will Do | ResponsibilitiesClinical Oversight & Whole-Person Support

• Conduct initial and ongoing clinical assessments, taking a holistic view of each individual's medical history, strengths, and goals.

• Design, implement, and routinely evaluate individualized nursing care plans tailored to individuals with complex developmental and medical needs.

• Monitor specialized medical equipment, oversee health protocols (including G-tube care, glucose monitoring, and seizure management), and evaluate health conditions in partnership with primary care physicians and specialists.

• Recommend evidence-based clinical interventions and participate actively in interdisciplinary team meetings alongside care managers, behavior analysts, and therapists.

• Provide clinical supervision, competency evaluations, and hands-on protocol training for Direct Support Professionals (DSPs) and natural supports, building their confidence and technical skills.

Crisis Consultation & Transition Guidance

• Provide expert clinical consultation to licensed team members during crisis situations, ensuring person-centered intervention.

• Review and evaluate crisis documentation, refining clinical recommendations to promote ongoing stability and safety.

• Partner with interdisciplinary teams to create smooth, safe transition strategies for individuals returning home following a crisis event.

Documentation & Compliance

• Maintain timely, accurate documentation for clinical evaluations, care plan updates, and consultation encounters in full alignment with federal, state, MCO, and HIPAA standards.

• Ensure complete accuracy within electronic health record (EHR) systems (e.g., OnTarget, ReadySign), completing progress notes, incident reports, and annual reassessments promptly.

What We Are Looking For | QualificationsExperience & Core Competencies

• Experience: Minimum 2–3 years of clinical nursing experience; experience supporting individuals with intellectual and developmental disabilities (I/DD) is highly preferred.

• Technical & EHR Skills: Proficiency in specialized medical instrumentation, electronic health records (Therap preferred), digital document systems (ReadySign), and HIPAA compliance workflows.

• Communication: Exceptional written and verbal communication skills, with a proven ability to convey clinical concepts compassionately and clearly to families, natural supports, and interdisciplinary team members.

• Background Screening: Must pass comprehensive criminal background checks and maintain a clean listing on the NC Health Care Abuse Registry.

Education & Licensure

• Education: Bachelor’s Degree or Advanced Degree from an accredited nursing program preferred

• Licensure: Current, valid Registered Nurse (RN) license in North Carolina (or valid compact state license acceptable in NC); board certification preferred.

• Waiver Knowledge: Familiarity with the NC Innovations Waiver is highly desirable.

• Regulatory Alignment: Commitment to maintaining compliance standards set by governing bodies in partnership with CCMS.

Location & Work Style

This is a hybrid role based in North Carolina, offering a balance of remote flexibility and targeted on-site or community visits based on service needs and family engagement requirements.
