Role Responsibilities

Education Requirements

• MD or DO from an accredited medical institution.
• Successful completion of an accredited Family Medicine residency program.

Certifications

• Board Certification in Family Medicine.
• Possession of (or eligibility for) an active Ohio medical license.
• BLS and ACLS certifications are preferred.
