Home Visiting Nurse: Family Support & Newborn Wellness

Central Jersey Family Health Consortium, Inc. is seeking a Family Connects Nurse to provide home visits to families with newborns in Middlesex County. This role involves assessing family needs in physical and psychosocial wellbeing and facilitating community agency referrals. The ideal candidate will have an A.S. degree in nursing or equivalent, a current RN license, and experience in maternal child health. Strong interpersonal skills and the ability to engage families in developing support plans are essential.
