Registered Nurse - Community Outreach

Role Overview: The Registered Nurse provides comprehensive nursing care during a patient's hospital stay, assessing, managing, and coordinating treatment plans in collaboration with the healthcare team. Key Responsibilities: Conducts thorough assessments, develops and evaluates individualized care plans, administers medications, and documents patient progress. Collaborates with team members, ensures effective communication, and provides patient and family education. Recognizes and responds to changes in patient conditions, manages specialized care such as restraint and pain management, and adheres to safety and infection control protocols. Qualifications & Skills: Valid RN license in the applicable state, current BLS certification, and ability to perform age-appropriate care for adult and geriatric patients. Demonstrates professionalism, teamwork, adaptability, and excellent communication. Maintains compliance with policies, participates in quality improvement initiatives, and fosters a respectful environment. Additional Duties: Ensures a safe environment, supports performance improvement activities, and delivers service excellence by maintaining courtesy, confidentiality, and patient-centered care.
