Professional Summary
Dedicated and relationship-focused Clinical Liaison responsible for developing and maintaining strong partnerships with healthcare providers, hospitals, physicians, case managers, discharge planners, and community organizations. Serves as a key connection between clinical teams, referral sources, patients, and families to facilitate appropriate referrals, admissions, and continuity of care. Combines clinical knowledge, communication skills, and business development strategies to strengthen referral relationships and support organizational growth.

Key Responsibilities

• Develop and maintain professional relationships with hospitals, physicians, skilled nursing facilities, rehabilitation centers, home health agencies, case managers, social workers, and other healthcare referral partners.

• Educate referral sources, patients, and families regarding available clinical services, programs, admission criteria, and care options.

• Identify and cultivate new referral opportunities while maintaining strong relationships with existing referral sources.

• Collaborate with interdisciplinary teams to evaluate patient needs and determine appropriate services and level of care.

• Coordinate referrals, admissions, and transitions of care to promote timely access to services.

• Communicate clinical and patient information accurately while maintaining confidentiality and compliance with HIPAA and organizational policies.

• Conduct facility visits, presentations, educational sessions, and community outreach activities to increase awareness of available services.

• Monitor referral activity, admissions, and outreach efforts and communicate trends and opportunities to leadership.

• Partner with nursing, social work, therapy, marketing, admissions, and administrative teams to ensure a seamless patient experience.

• Support discharge planning and continuity of care by facilitating communication among patients, families, providers, and care teams.

• Maintain accurate documentation and follow applicable healthcare regulations, accreditation standards, and organizational procedures.

• Represent the organization professionally and serve as a knowledgeable resource to healthcare providers and community partners.
