Shifts: Monday to Friday 8am - 5pm

Seven Healthcare are seeking an experienced RN for a Case Management travel assignment in Forest Lake, MN. This role focuses on hospice care within a supportive work environment, ensuring comprehensive care plans for patients.

Key Responsibilities of the RN - Case Management

Perform comprehensive assessment of hospice patients.
Collaborate with primary care providers to ensure the implementation of individualized, comprehensive care plans.
Address, identify, and continuously reassess cost-efficient, appropriate levels of care.
Put non-medical support in place to ensure compliance with treatment plans.
Coordinate care transitions and engage specialty resources as needed.
Keep detailed records of clinical, functional, and fiscal outcomes.
Identify opportunities for health promotion and illness prevention.
Act as a patient advocate by protecting privacy and confidentiality issues.

Requirements of the RN - Case Management

Associates or Bachelors Degree in Nursing or related field.
Current, unrestricted state Registered Nurse license.
Ability to analyze complex medical information and make rational decisions.
Hospice Experience.
