Health Social Worker LMSW - Outpatient Cystic Fibrosis Program

Location 85 Jefferson Street Hartford (10068) Job Summary Hartford Hospital seeks a full‑time outpatient Health Social Worker for the Cystic Fibrosis Program. This role delivers care based on comprehensive biopsychosocial‑spiritual assessments as part of a collaborative, interprofessional team. The social worker supports patients and families through integrated mental health coordination. Requires an activated and engaged social worker exerting discretionary effort and demonstrates strong advocacy skills. A Social Worker that values holistic patient‑centered and family‑focused care. Job Responsibilities Maintains ethical principles and professional standards of practice in adherence to NASW Code of Ethics Recognizes and addresses health disparities‑inequalities and injustices directed toward clients, related to access to care and provision of health services Develops and maintains cultural competence with an understanding of the history, traditions, values, and family systems of client groups as they relate to health care and decision‑making. Displays a sensitivity to and awareness of the diversity in cultural groups and integrates this knowledge into practice Independently provides ongoing assessment, including gathering comprehensive information to use in developing interventions and treatment strategies Independently implements intervention and treatment plans that promote client well‑being and ensure a continuum of care.
