RN Home Care Supervisor

RN Field Registered Nurse

Comfort Keepers in Montclair, Hasbrouck Heights & Parsippany NJ is looking for highly motivated individual/self-starter for RN client care coordinator duties with a potential to grow into other roles (care manager, DON, nurse liaison) if so desired.

Our clients are in Essex, Bergen, Passaic & Morris County towns of Short Hills, Millburn, Maplewood, Montclair, Caldwell, Livingston, Verona, South Orange, West Orange, Essex Fells, East Hanover, Roseland, Clifton, Passaic, Union, Rutherford, Carlstadt, Hackensack, Parsippany, Fort Lee, Leonia, Palisades Park, Hasbrouck Heights, Montville & Wood-Ridge.

You must be licensed RN in the state of New Jersey (Bachelors degree preferred).

Responsibilities include:

• Performing an initial in-home evaluation of clients and putting together a plan of care.
• Documenting and initiating the clients Service Plan in our CRM system.
• Re-evaluating existing clients in their homes - reassessments 30/60/90.
• Supervisory visits to check whether the assigned caregivers are meeting the clients needs.
• Performing Competency tests & monitoring of our caregivers & home health aides.
• Managing client and caregiver files compliant per Board of Nursing & Accreditation requirements.
• Perform care coordination intake as well as care escalation resolution.
• Participate in weekly on-call rotation.
• Need to be a multi-tasker, home care experience preferred.
