Community Health WorkerUnder general supervision, performs routine community health worker duties for the Babies Born Healthy Program, provides care coordination, case management, conducts virtual and in-home interviews (as needed) and virtual telehealth visits to pregnant and post-partum women, providing care coordination and perinatal navigation services, connecting women to resources, information and distributes supplies.Provides care coordination, health education, identifies resources, and conducts telephone and in-home perinatal navigation services for pregnant and postpartum women in the program.Conducts interviews and performs screenings for high-risk pregnant and postpartum women to identify needs, link to resources in Baltimore County and provide education on prenatal and postpartum topics.Coordinates with community partners and agencies for available inter-agency community programs and services. Assists and links clients and families with resources.Completes documentation, encounter forms and data tracking.Maintains cooperative and liaison relationships with public and private agencies.Examples of other duties:Performs other related duties as required.Required qualifications:Possession of a high school diploma or appropriate equivalent plus at least one year experience working with the public.Knowledge, skills, and abilities:Knowledge of English grammar. Knowledge of and sensitivity to cultural influences.Ability to learn and follow county and departmental rules, regulations, policies, and procedures; ability to establish effective working relationships with clients, families, and public and private agencies; ability to communicate effectively with individuals and groups; ability to work flexible hours; ability to work in a variety of environments; ability to maintain documentation regarding activities; ability to maintain records, files, and reports; ability to coordinate activities; ability to maintain inventory of supplies.Licenses or certificates:Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.Proof of licenses, certifications and education:Applicants are required to submit proof of licenses, certifications and education beyond high school to meet the required and preferred qualifications of the position. Diplomas or transcripts must show the applicant's major field of study. Copies and unofficial transcripts are acceptable.Proof of degree equivalency:Applicants who have obtained a degree from outside the United States of America are required to submit degree equivalence documentation from World Education Services, Inc.Examination procedure:Applicants will be qualified based on an evaluation of their training and experience, as stated on their application, which includes answers to the supplemental questions. Applicants must state the dates and duties of past and present experience clearly and completely for evaluation purposes.
