Advocate Health Care is seeking a Registered Nurse (RN) to lead hospice admissions across Oak Brook, Lombard, Park Ridge, Elgin and surrounding areas. Shift options include day/PM, with a schedule Monday, Thursday, Friday and occasional Tuesday/Wednesday, plus one weekend per month and two holidays per year. You will assess patients, coordinate with the interdisciplinary team, obtain prognosis and informed consent, review insurance coverage, and educate families about hospice services. #J-18808-Ljbffr Advocate Health Care
