Travel MSW – Medical Social Worker / Case Management
Pay: Up to $62/hour ($2,480/week at 40 hours )
Start Date: November 16, 2026
Contract Length: 13 weeks
Positions Available: 1
Schedule
- Shift: Day shift, 8:00 AM–4:30 PM
- Schedule: 5 × 8-hour shifts per week
- Expected Workdays: Sunday–Thursday
- Days off may vary based on staffing.
- Floating: Laguna facility may be required.
- Caseload: Approximately 20–23 patients
- Scrub Color: Turquoise
Job Description
Providence Mission Hospital is seeking an experienced Medical Social Worker (MSW) for its inpatient Case Management department. This role focuses on patient assessments, discharge planning, and coordinating post-acute care services for adult and geriatric patients.
Responsibilities:
- Complete initial patient assessments within 24 hours of admission.
- Conduct biopsychosocial assessments and evaluate patient needs.
- Develop and coordinate individualized discharge plans.
- Arrange referrals to rehabilitation facilities, skilled nursing facilities, and home health services.
- Coordinate durable medical equipment (DME) and post-discharge care.
- Collaborate with physicians, nurses, insurance case managers, and interdisciplinary teams.
- Connect patients with community resources.
- Maintain accurate documentation in Epic.
Requirements
- Master's degree in Social Work (MSW).
- Minimum 2 years of professional experience.
- At least 1 year of acute-care inpatient hospital experience.
- Recent hospital experience within the last 3 months.
- Previous travel assignment experience required.
- Epic experience mandatory.
- BLS certification required.
- LCSW preferred.
- Case management and discharge planning experience.
