OrMaster's Degree in Physical Therapy, Occupational Therapy, Speech Language Pathologist or Audiology; from an accredited program. OrPh.D. in Physical Therapy, Occupational Therapy, Speech Language Pathologist or Audiology; from an accredited program.Upon hire: Oregon Physical Therapist License. OrUpon hire: Oregon Occupational Therapist License. OrUpon hire: Oregon Speech Language Pathologist License. OrUpon hire: Oregon Speech Language Pathologist Permit.Upon hire: National Provider BLS - American Heart Association.Upon hire: In compliance with company policy and state regulations, a Department of Human Services (DHS) criminal background check is required for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
