We are seeking a Caregiver / Personal Care Assistant to provide in-home support for an elderly male client. The caregiver will primarily report to the client’s spouse and assist with mobility, personal care, transportation, errands, established diabetes care routines, and companionship while promoting the client’s safety, dignity, privacy, and independence.

 

Responsibilities

• Assist with safe wheelchair transfers between the bed, wheelchair, and vehicle.
• Provide mobility assistance throughout the home and community.
• Transport the client to medical appointments, grocery stores, pharmacies, and approved errands.
• Assist with grocery shopping, including selecting, purchasing, transporting, and storing groceries.
• Provide appropriate support with the client’s established diabetes care routine within the authorized scope of duties.
• Assist with medical bandage care and dressing changes as directed by the healthcare provider and permitted by training and scope of practice.
• Assist with personal care and activities of daily living as needed.
• Observe and report changes in the client’s condition, incidents, falls, injuries, or other concerns to the client’s spouse or designated responsible party.
• Maintain a clean, safe environment and keep transfer areas free of hazards.
• Provide companionship and emotional support while following the care plan, household instructions, and safety procedures.
• Document services provided and maintain the client’s privacy, dignity, and independence.

 

Required Qualifications

• At least one year of personal caregiving experience.
• Active and valid driver’s license.
• Reliable personal vehicle, current vehicle registration, and required automobile insurance.
• Ability to provide transportation and safely assist with vehicle transfers.
