RN/LPN Full Time (32 hours/week; 3p-11p) – $3000 Sign on Bonus RiverWoods is currently seeking a Full Time (32 hours/week) RN or LPN to provide direct resident care in a team‑focused environment where our residents are at the heart of what we do. Collaborate with interdisciplinary team members to formulate individualized resident care plans, update plans as necessary, and participate in Resident Care Plan Meetings. Institute care protocols such as falls prevention, safety and elopement, behavior management, weight management, skin integrity, bowel/bladder management, infection control guidelines (Standard Precautions), and pain assessment and management. Assess and document changes in residents’ physical or mental status and report findings to the primary care physician and treatment team, providing complete clinical data to facilitate decision‑making. Provide leadership and direction for direct caregivers, prepare daily assignments, delegate workload, and supervise direct care through observation and evaluation. Establish positive working relationships with team members, residents, and family members to facilitate open, direct communication. Obtain, administer, and document medications and treatments in accordance with professional guidelines and nursing department policies; maintain inventory and a locked system for narcotics. Support, create, and maintain a positive culture for team members and residents. Qualifications Must possess a Nursing Degree or Diploma from an accredited college, university, or nursing school. Long‑term care facility experience and specialized knowledge of nursing care for the elderly and rehabilitative nursing is preferred. Must hold a registered nursing (RN, LPN) license within the State of New Hampshire. Certification in intravenous administration preferred. Must maintain CEU’s annually. CPR is required.
