Independent Adult Day Centers Independent Adult Day Centers Position : Director of Medical Services | Reports to : Executive Director/Regional/Assistant Reginal Director of Medical Services Job Description The Director of Medical Services (DMS) is a licensed nurse and is responsible for all aspects of our Guests’ medical care during their time in our Center. The DMS performs a health assessment on each Guest and executes the medical plan of care for each Guest to ensure the optimal level of independence and well-being. The DMS provides direct medical care and assists with Activities of Daily Living (ADL) care provided by other Guest staff. The DMS is responsible for record keeping for all medical aspects of Guest care. The DMS may work with other IADCC medical staff on Guest Health Care Coordination (HCC) and may participate in non-medical Guest care such as outings and activities. Essential Functions: Minimum Required: Perform head to toe physical assessments of current and prospective Guests upon admission and annually. Develop and implement medical plan of care. Completion and documentation of regular monitoring of vital signs/weight/blood sugar, medication administration, bowel and bladder care, and education. Develop and implement with Guests, as needed, programs in bowel and bladder care, restorative feeding, ambulation, range of motion, personal living skills, transfers, and showering. Update and maintain Guest medical files, including medical plan of care, care records, and progress notes. Assist Guests and Supervise Guest staff in assistance with ADLs, including eating/restorative feeding, ambulation, range of motion, transfers, toileting, and showering. Ensure accurate record-keeping of ADL care. Observe and report to Regional/Assistant Regional Director of Medical Services and/or Center Executive Director conditions, reactions, and responses of Guests as related to assigned duties. Assemble, disassemble, and clean equipment and accessories. Always maintain a safe environment. Always maintain confidentiality. Ensure Guest privacy and dignity in all manner of care. Ensure security of, and proper documentation for all Guest medication stored and administered at the Center. Assist with the maintenance of licensure and accreditation standards. Collaborate with other IADCC medical staff on Guest Health Care Coordination, including medical record consolidation and medication management. Administer PPD (TB) screening and drug tests (prospective employees). Assist in keeping bathing room, shower room, and exam room neat and clean. May be required to perform heavier cleaning/mopping of all Center areas. Assist with, attend, and/or lead non-medical Guest care including activities and outings, as needed. Ensure toileting, feeding, and bathing needs are met, as required by specific guests. Assist with training and onboarding of new Guest care staff. Ensure all Guest care staff know how the plan of care for each Guest current and new admissions. Complete incident/accident forms as required when needed. Complete BI-Weekly DMS Report every other Friday Attend and actively participate in morning meeting stand-up, leadership, and Guest Caregiver meetings. Other duties as needed. Qualifications LPN or RN license Direct care experience working with geriatric and/or adults diagnosed with Intellectual Disabilities preferred. Valid driver's license in state of residence Qualifying criminal background and driving record. Certification/experience in certain areas of training including but not limited to CPR, First Aid, non-violent physical interventions, behavior management, medication administration, and driving Ability to calmly and repetitively model appropriate behavior in difficult or challenging situations Ability to react to situations, i.e., rescue, preventing a resident from hurting themselves Ability to work with frequent interruptions Essential Physical Requirements The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
