Travel Physical Therapist | Stockton, California

Provide specialized physical therapy services in an outpatient hospital setting, focusing on comprehensive patient evaluations and individualized treatment plans. This travel position requires clinical expertise in physical therapy to support patients' functional recovery and community reintegration.


Responsibilities:

• Conduct thorough evaluations of assigned patients, developing and adapting treatment plans that align with patient goals and physician directives.
• Delegate and oversee discipline-specific therapy services, ensuring effective program design for optimal patient outcomes.
• Educate patients, families, students, and coworkers to foster understanding of care processes and promote rehabilitation success.
• Maintain timely, clear, and accurate documentation, including goal-setting, medical necessity letters, and quality assessments, complying with all regulatory requirements.
• Uphold patient and employee safety by adhering to established protocols for safe patient handling, infection control, and workplace hazard identification.
• Participate in student training programs as appropriate, providing supervision and evaluation.
• Engage in ongoing professional development through continued education and active involvement in professional organizations.
• Comply with discipline-specific State Practice Acts, maintain licensure, and perform duties ethically and professionally.
• Support organizational performance improvement initiatives by contributing feedback and following through on action plans.
