Job Summary With 114+ years of service to the community, the Natick Walpole Visiting Nurse Association is one of the most trusted non-profit providers of home health, public health, and community- based certified health home care in the region. We are looking for a compassionate, reliable PT to join our team to provide excellent care and who enjoys working with patients in their homes. Openings for 2 to 3 Physical Therapists for coverage areas for Natick, Ashland, Framingham, Wayland, Weston, Wellesley, Needham, Dover, Sherborn Responsibilities Provide care and services in accordance with the plan of care. Assist community members and health field personnel to assess, plan for, and provide needed health and related services. Document care provided in an accurate and timely manner. Provide teaching to clients and families. Coordinate and collaborate with other members of the health care team, community agencies, and referral sources. Communicate the plan of care with patients and families and ensure understanding of the exercise program prescribed. Responsive to meeting or exceeding patient/client expectations and promotes a positive image of NWVNA. Adhere to HIPAA laws, maintains patient confidentiality, and adheres to all agency policies and procedures. Attend continuing education programs. Other duties assigned by the department manager. Requirements/Qualifications Current licensure from the Board of Allied Health in the State of Massachusetts Graduate of an accredited college of physical therapy Must have demonstrated competence in community health or clinical patient care setting, providing direct patient care with patients for a minimum of two years. Associate’s degree and graduation from an accredited Physical Therapy program.
