Care Manager (RN)

Job TitleSummary: This position offers remote and in-person work modes. Candidates must reside in North East Ohio, preferably close to Trumbull, Stark, Seneca, or Portage counties. The duration is six months with possible extension and/or conversion. Training is scheduled for Monday to Friday, 8 a.m. to 5 p.m. EST, over five weeks, both in classroom and virtual formats with cameras on and no time off. The working shift is Monday to Friday, 8 a.m. to 5 p.m. EST.ResponsibilitiesDevelop, assess, and facilitate complex care management activities for members with primarily physical needs.Provide high-quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families.Log on to laptop by 8 a.m., review emails, tasks, and voicemails, and respond to any urgent needs.Review calendar for possible meetings and/or scheduled visits.Arrange 5-7 in-home visits weekly; documentation must be completed within 24 hours.Reimbursed for mileage.RequirementsDegree from an accredited school of nursing.Minimum two years of related experience in case management or home health experience.Ohio RN licensure required.Reliable vehicle for weekly travel.Preferred SkillsComputer literate with knowledge of Microsoft Excel, Word, Teams, Outlook, OneNote, OneDrive, PowerPoint, Explorer, Chrome.Critical thinker.Works well independently and possesses strong troubleshooting skills.Personable with strong communication skills.Ability to think critically, be flexible, open to change, and work well in a team.AXEL01
