RPh on the Go is hiring a Home Infusion Pharmacist for a 13-week contract opportunity in San Antonio, Texas. This role is ideal for a pharmacist with sterile compounding or strong clinical pharmacy experience who is comfortable managing infusion therapies and monitoring patient outcomes.

Schedule: 13-week contract. M-F 12:00 PM–8:00 PM

Responsibilities

• Evaluate patients for home infusion therapy
• Develop and monitor individualized medication therapy plans
• Review laboratory values and apply pharmacokinetic principles
• Prepare, verify, and dispense sterile medications in accordance with USP standards
• Counsel patients and caregivers on medications and infusion administration
• Collaborate with healthcare providers to ensure continuity of care
• Manage infusion pumps and medication administration equipment
• Maintain accurate documentation and support medication safety and regulatory compliance

Requirements

• PharmD or graduate of an accredited School of Pharmacy
• Active and unrestricted Texas Pharmacist license
• Strong clinical and patient counseling skills
• Home infusion or sterile compounding experience preferred

Candidates should also have qualifying pharmacy experience through sterile IV compounding, compounding/MTM, residency, APPE, or equivalent pharmacy experience .
