Givens Estates is looking for a full-time Social Worker to join our Resident Services team in supporting our Skilled Nursing residents. This position’s primary function is to meet the psychosocial needs of residents as identified by staff and residents. This position will relate to the Givens Estates Health Center and Wood Assisted Living Social Workers as it relates to resident level of care changes and coordination of such.

What you'll do:

• Work in collaboration with other Independent Living Social Work Navigators, the Givens Estates Health Center and Assisted Living Social Work Navigators, Nurse Navigator, and Spiritual Care chaplain
• Implement and evaluate planned social services activities, both on group levels and in one-to-one settings
• Share Plans of Care and get input from residents and/or their designated others
• Provide referral and community resource information to residents/families, as needed, i.e. financial assistance program
• Develop supportive and caring relationships with residents and provide support and counsel residents during crisis/death of spouse and friends
• Facilitate resident support groups and other educational presentations in collaboration with Life Enrichment
• Promote the Wellness initiatives of Givens Estates. Develop and coordinate special services for and with residents with the understanding that residents are whole persons
• Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor

What you'll need:

• Bachelors of Social Work (BSW) or Master of Social Work (MSW) or a degree in a human services-related field
• 2-4 years working experience with an aging adult population or any equivalent combination of training
• Possesses working knowledge of the healthcare system and various levels of care
• Demonstrated proficiency in time-management to effectively address programming, implementation, and counselling requirements of residents

Compensation for this is based on background and experience:

$28 - $32 for BSW

$65,000 - $68,000 for MSW
