Field-Based Case Manager II — Mental Health & Housing

JWCH Institute, Inc.

JWCH Institute, Inc. is seeking a Case Manager II in Los Angeles, CA to deliver comprehensive case management for the DMH-funded outpatient program. You will conduct intake, assess, develop care plans, coordinate services, and support housing stabilization while working with a multidisciplinary team. The role requires 1 year of relevant experience (or 3 years with a high school diploma), knowledge of trauma-informed care and harm reduction, and EHR proficiency. #J-18808-Ljbffr JWCH Institute, Inc.
