Description We are currently looking for a Housing Specialist for our HOPWA program. This position will be housed out of our Opportunity Center Monday through Friday 8 AM to 5 PM. As a Housing Specialist, you will facilitate eligibility screening and housing placement for individuals and families affected by HIV/AIDS, assist residents in achieving housing stability within a continuum of housing services, provide assessment, placement, and follow‑up support services to the target population, and function as an integral part of an interdisciplinary team to develop individual housing plans. Responsibilities Perform intakes, screening and assessments on potential and current residents. Meet with residents in transitional housing 2 times a month or more to assess needs and ensure compliance with program agreements. Oversee hygiene and safety standards of all units; inspect apartments, smoke detectors and fire extinguishers; review emergency escape plans and unit upkeep with residents. Identify unmet needs of residents, provide service engagement, linkages to services, coordinate referrals and follow‑up. Facilitate procedures for entering and exiting the program and prepare units. Maintain housing/health and safety standards of all properties and master‑leased units. Consult with internal staff and external case managers/service providers to establish goals and objectives; participate in internal and external staffing. Develop individualized housing plans with residents. Intervene in crisis situations and coordinate with other supports as needed. Consult with landlords and management companies regarding responsibilities to units and ensure that repairs are made. Understand the Arizona Landlord‑Tenant Act and its application to residents and units. Participate in community planning, advocacy and networking with other providers as directed by supervisor. Collaborate with all Project HHHOME providers and property site management. Facilitate annual recertification; maintain HQS housing habitability standards for all units; follow income and rent procedures and ensure resident lease compliance. Share emergency on‑call duty as required. May be expected to drive a SB&H vehicle to transport clients, per the discretion of the clinic and/or program. Education and Qualifications Must be at least 21 years of age. Have a high school diploma or GED and at least two years of behavioral health or integrated healthcare work experience; OR Have an associate's degree in an unrelated field and at least one year of full‑time behavioral health or integrated health care work experience; OR Have a related associate’s degree and at least six months of behavioral health or integrated healthcare work experience; OR Hold a valid Arizona driver’s license and be able to pass a motor vehicle screen for insurance purposes in order to transport clients. Have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight weeks of hire. Pass a 39‑month MVR report.
