Evaluate residents, develop and implement individualized treatment plans, and monitor progress toward functional goals. Deliver evidence-based interventions and adaptive equipment training to promote independence and safe functional performance. Collaborate with the occupational therapist, nursing staff, therapy teammates, and the interdisciplinary team to coordinate care and discharge planning. Document evaluations, treatment plans, progress notes, and discharge summaries in the resident record; ensure timely, accurate, and compliant documentation. Ensure patient safety, infection control, proper use of assistive devices, and adherence to facility policies during treatment sessions.
