Geriatric NP: Impactful Long-Term Care Clinician

CareMed Health Solutions is seeking a Nurse Practitioner to deliver primary and preventive care to residents in long-term care facilities in Ohio. You will manage acute changes, engage in advanced care planning, and coordinate with an interdisciplinary team to optimize resident wellbeing.
