Details Home Health setting covering Seaside, OR and surrounding areas Day shift schedule, 8-hour shifts (facility cannot accommodate 4x10s or 3x12s) 1:1 patient ratio with up to 6 patient visits per day, 30 productivity points standard No weekend requirements No on-call requirements No floating between like units required Uses EPIC Remote Client for Home Health documentation (laptop) and Rover app/Oasis for charting Oasis experience required Requirements Minimum 1 year of skilled Home Health experience required BLS certification required License required if applicable Compact state license must be in hand at time of submission Must have reliable personal transportation for daily travel between patient visits Primary series COVID vaccination plus one booster required Additional Information Main responsibilities include home exercise program development and patient/family education Patient population includes orthopedic, neurologic, and post-surgical conditions such as total joint replacements, spinal conditions, stroke rehabilitation, and traumatic brain injury Business casual attire required; scrubs allowed but not mandatory (no jeans) Free parking available Mileage reimbursed at IRS standard rate System groups clients to minimize drive time; occasional maximum daily travel radius of about 60 miles (60-90 minute total drive time) First-time travelers accepted Local candidates accepted within a 50-75 mile radius at the same pay rate Former Providence employees may return as travelers after a 3-month waiting period Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) Two supervisory references covering 1 year of employment within the last 3 years required American Traveler Job ID P-717681.
