Licensed Practical Nurse (LPN)

The primary functions of a Licensed Practical Nurse (LPN) involve assisting with the assessment, planning, implementation, and evaluation of care for all patients, across all age groups, during their shift. This role requires adherence to established standards of skilled nursing practice and collaboration with the care team while advocating for patients. The LPN effectively provides person-centered care and services.

Responsibilities

• Administer medications and treatments under the supervision of a Registered Professional Nurse in accordance with facility policy.
• Monitor and document patient vital signs including respiration, temperature, blood pressure, and heart rate, and report any changes in patient/resident conditions.
• Practice ethically and professionally within the scope of capabilities and advocate for patients.
• Maintain clinical competence specific to the care area, including required education within the stated timeframe.
• Coordinate patient care with the interdisciplinary team while keeping nursing personnel and RN informed regarding changes in patient conditions.
• Document appropriately and thoroughly in the Electronic Medical Record (EMR) or Point Click Care.
• Participate in interdisciplinary care and meetings as required.

Schedule

This position is for a 37.5 hours per week schedule, working evenings from 3:00 PM to 11:30 PM, with every other weekend and holiday required. Staffing ratios for this shift are 2 LPNs to 3-4 CNAs for 40 residents.

Required Experience / Certifications / Licensure

• Valid state licensure as a Licensed Practical Nurse (LPN).
• Experience with Point Click Care is preferred.
• Completion of health assessments such as MMR, TB test, Hepatitis B, and Tdap as specified.
