Registered Nurse Care Manager
Advocate Health Care
Advocate Health Care is seeking a registered nurse to lead discharge planning and care coordination across the hospital team. You will assess patient needs, develop appropriate plans, and coordinate services with families, physicians, and social work to ensure timely, safe transitions. In this role you will perform utilization reviews, document care progression per CMS guidelines, and collaborate with case management to address barriers to discharge while maintaining regulatory compliance and #J-18808-Ljbffr
