About the Role

We are seeking a Registered Dietitian to join our nutrition team, providing expert guidance and support to our residents, families, and staff. As a Registered Dietitian at Humboldt House, you will be responsible for:

• Conducting nutritional assessments and developing individualized nutrition plans
• Providing medical nutrition therapy to residents with various dietary needs
• Collaborating with our interdisciplinary team to ensure seamless care coordination
• Educating residents and families on healthy eating habits and disease management
• Maintaining accurate records and documentation of resident care

Responsibilities

As a Registered Dietitian at Humboldt House, your responsibilities will include:

• Conducting comprehensive nutritional assessments and developing individualized care plans
• Implementing and monitoring resident nutrition plans, making adjustments as needed
• Collaborating with our dietary team to ensure meal planning and menu development align with resident needs
• Participating in multidisciplinary team meetings to discuss resident care and goals
• Staying current with industry developments and best practices in nutrition and dietetics
