Responsibilities: Deliver comprehensive primary care at home/senior communities visits to patients Represent Bloom Healthcare in the St Louis market as a Bloom ambassador supporting our new market growth including market analysis to understand the local healthcare landscape and competitors. Strategic planning to grow your patient panel and deliver comprehensive primary care at home visits to 10 patients per day. Build trusted relationships by engaging with key members of our patients’ care teams, including assisted living facilities, hospitals, clinics, and community organizations, to facilitate coordination of care. Collaborate with interdisciplinary teams, including specialists, pharmacists, and social workers, to coordinate care, address complex patient needs, and achieve optimal health outcomes. Provide data-driven, condition-specific specialty visits to patients as needed Manage the health outcomes of an empaneled patient population Engage with and appropriately utilize population health data to drive superior patient outcomes Attend ongoing educational opportunities Commute to assigned assisted living communities to provide care at home Occasional travel to Denver market for initial and ongoing training Physician Benefits: Scheduled Days: Monday - Friday (3, 4, or 5 days) Relocation assistance packages available Flexible schedules with rotating call (2-3x yr) Approximately 10 visits per day 100% covered employee Health, Dental and Vision insurance on Day 1 401k Employer paid Basic Life and AD&D policies $350 Monthly Auto Allowance 3 weeks PTO & 6 paid holidays Company cell phone & tablet Licensing fees and liability insurance coverage A CME yearly allowance Physician Qualifications: Appropriate Licensure in the state of Missouri Bilingual preferred but not required Our Physicians are supported by an excellent staff of MD’s, Nurse Practitioners, telephone triage, marketing ambassadors, IT, billing and scheduling personnel. The pay range for this role in the state of Missouri typically falls between $225,000 - $260,000 annually. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
