Salary: $100,000 - $150,000/Annual
The primary purpose of this job is to plan, develop, and direct the overall operation of the nursing services department in accordance with the current, Federal, State, Local standards, and accrediting organization's guidelines, and regulations that governs the CLHF, and may be directed by the administrator to ensure that the highest degree of quality care is maintained at all times.
Job Responsibilities
- Plan, develop, organize, implement, evaluate, and direct the facility's programs and activities in accordance with guidelines issued by the governing board.
- Plan, develop, organize, implement, evaluate, and direct the nursing service department,as well as its programs and activities, in accordance with current rules, regulations, andguidelines that govern nursing care facilities.
- Develop, maintain, and periodically update written policies and procedures that governthe day-to-day functions of the nursing service department.
- Develop, maintain, and periodically update the Nursing Service Procedures Manual andnursing service objectives and philosophies.
- The Director of Patient Care Services/Director of Nursing shall be available forconsultation and able to come into the CLHF within 30 minutes, if necessary if noregistered nurse is on duty.
- The Director of Patient Care Services/Director of Nursing shall visit each patient at leasttwice a week for approximately two hours, or more as patient care requires.
- Develop, maintain, and periodically update written job descriptions for each level ofnursing personnel in accordance with pertinent laws and regulations.
- Develop methods for coordination of nursing services with other resident services toensure the continuity of the residents' total regimen of care.
- Participate in facility surveys (inspections) made by authorized government agencies asnecessary or as may be directed.
- Develop QAPI program and implement appropriate plans of action to correct identifieddeficiencies.
- Participate in CLHF required committees (Patient Care Policy Committee, InfectionControl Committee, and Pharmaceutical Service committee).
- Perform administrative duties such as completing medical forms, reports, evaluations,studies, charting, etc., as necessary.
- Assist in calculating the number of direct nursing care personnel on duty each shift.
- Report such information to the Administrator or his/her designee to ensure that accuratestaffing information is posted.
- Participate in regularly scheduled reviews of the Discharge Plans and make such plansavailable to the Care Plan Committee as required or as may be necessary.
- Determine the staffing needs of the nursing service department necessary to meet thetotal nursing needs of the residents.
- Assist the Administrator and/or the HR staff in the recruitment and selection of nursingservice personnel.
- Ensure that all nursing personnel meet the education and experience requirements.
- Ensure that all nurse aide trainees are under the direct supervision of a licensed nurse.
- Develop, maintain, and periodically update the written procedure for ensuring thatprofessional nursing personnel, including private duty nurses, have valid and currentlicenses as required by this State.
- Review complaints and grievances made or filed by department personnel.
- Ensure that departmental disciplinary action is administered fairly and without regard torace, color, creed, national origin, age, sex, religion, handicap, or marital status.
- Report occupational exposures to blood, body fluids, infectious materials, and hazardouschemicals in accordance with the facility's policies and procedures governing accidents and incidents.
- Terminate employment of personnel when necessary, documenting and coordinating suchactions with the HR Director and/or Administrator.
- Report known or suspected incidents of fraud to the Administrator.
- Ensure that departmental computer workstations left unattended are properly logged offor the password protected automatic screensaver activates within established facilitypolicy guidelines.
- Review nurses' notes to ensure that they are informative and descriptive of the nursingcare being provided, that they reflect the resident's response to the care, and that suchcare is provided in accordance with the resident's wishes.
- Monitor medication passes and treatment schedules to ensure that medications are beingadministered as ordered and that treatments are provided as scheduled.
- Develop, implement, and maintain an effective orientation program that orients the newemployee to the facility, its policies and procedures, and to his/her job position andduties.
- Provide In-service training programs ( e.g., OSHA, TB, HIP AA, Abuse Prevention,
- Safety, Infection Control, etc.) and in-service related to nursing service.
- Ensure that all personnel performing tasks that involve potential exposure to blood/bodyfluids participate in appropriate in-service training programs prior to performing suchtasks.
- Ensure that all resident care rooms, treatment areas, etc., are maintained in a clean, safe,and sanitary manner.
- Ensure that nursing service personnel participate in and conduct all fire safety and disaster preparedness drills in a safe and professional manner.
- Ensure that nursing service personnel follow established infection control procedureswhen isolation precautions become necessary.
- Develop, implement, and maintain a procedure for reporting hazardous conditions or equipment.
- Ensure that personnel wear and/or use safety equipment and supplies ( e.g., back brace,mechanical lifts, etc.) when lifting or moving residents.
- Ensure that a stock level of medications, medical supplies, equipment, etc., is maintainedon premises at all times to adequately meet the needs of the resident.
- Develop and implement procedures for the safe operation of all nursing serviceequipment.
- Ensure that only trained and authorized personnel operate the department's equipment.
- Ensure that personal protective equipment used in the handling of infectious materials is available and easily accessible to nursing service personnel.
- Develop a written plan of care (preliminary and comprehensive) for each resident thatidentifies the problems/needs of the resident, indicates the care to be given, goals to beaccomplished, and which professional service is responsible for each element of care.
- Ensure that all personnel involved in providing care to the resident are aware of the resident's care plan. Ensure that nursing personnel refer to the resident's care plan prior to administering daily care to the resident.
- Review nurses' notes to determine if the care plan is being followed.
- Review and revise care plans and assessments as necessary, but at least quarterly.
- Maintain the confidentiality of all resident care information including protected healthinformation. Report known or suspected incidents of unauthorized disclosure of suchinformation.
- Ensure that all nursing service personnel are knowledgeable of the residents'responsibilities and rights including the right to refuse treatment.
- Monitor nursing care to ensure that all residents are treated fairly, and with kindness,dignity, and respect.
- Review complaints and grievances made by the resident and make a written/oral report tothe Administrator indicating what action(s) were taken to resolve the complaint or grievance. Follow facility's established procedures.
- Maintain a written record of the resident's complaints and/or grievances that indicates theaction taken to resolve the complaint and the current status of the complaint.
- Report and investigate all allegations of resident abuse and/or misappropriation ofresident property.
- Ensure that nursing staff personnel honor the resident's refusal of treatment request.
- Ensure that such requests are in accordance with the facility's policies governing advancedirectives.
- Ensure that the facility's policies and procedures governing advance directives arereviewed with the resident and/or representative.
Requirements and Qualifications
- A Registered Nurse, a SNF administrator, or the licensee.
- Must possess a Nursing Degree from an accredited college or university.
- Bachelors degree in Nursing or other health care preferred.
- Must possess a current license from the State of California.
- Shall have at least two (2) year experience within the last five years in acute, SNF,
- Hospice, Home Health, primary care clinic or health facility.
- Supervisory experience preferred.
Physical Demands
Job may require considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient. Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet. It may be necessary to assist in the evacuation of residents during emergency situations.
Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position.
