Position Summary

We are seeking a compassionate and skilled Physical Therapist (PT) to join our Home Health team serving patients throughout San Joaquin or Stansilaus County. In this role, you will provide individualized physical therapy services to patients in their homes, helping them improve mobility, restore function, manage pain, and maximize independence.

As a member of an interdisciplinary care team, you will perform comprehensive assessments, develop treatment plans, provide therapeutic interventions, educate patients and caregivers, and collaborate with physicians and other healthcare professionals to achieve optimal patient outcomes.

This position offers clinical autonomy, scheduling flexibility, and the opportunity to build meaningful relationships with patients while delivering high-quality care in a home-based setting.

Essential Duties and Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities, functional limitations, and rehabilitation needs.

• Develop, implement, and modify individualized treatment plans based on patient goals and clinical findings.

• Provide skilled physical therapy interventions in accordance with physician orders and applicable regulations.

• Educate patients, families, and caregivers on therapeutic exercises, mobility techniques, safety measures, and injury prevention.

• Monitor patient progress and adjust treatment plans as clinically appropriate.

• Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team.

• Maintain timely, accurate, and complete clinical documentation in accordance with company policies and regulatory requirements.

• Participate in case conferences, care planning, and quality improvement initiatives.

• Comply with all applicable federal, state, and local laws, regulations, and professional standards.

 

Minimum Qualifications

• Current and unrestricted California Physical Therapist license.

• Minimum one (1) year of professional Physical Therapist experience.

• Minimum one (1) year of Home Health experience preferred.

• Current BLS/CPR certification.

• Valid California driver's license, reliable transportation, and automobile insurance as required by law.

• Ability to travel throughout assigned service areas.
