\nPACE of Montgomery County, Gaithersburg, MD
\nNurse Practitioner - Full Time (40 hours)
\nThe primary purpose of your job position is to provide primary medical management in collaboration with the PACE Organization (PO) Physician(s). Performs assessments in collaboration with the physician and as needed. Develops and implements appropriate plans of care to PO participants. Evaluates participant physical complaints and provides appropriate treatment. Provides participants and caregiver teaching and education. Functions as a member of the Interdisciplinary Team (IDT). Regularly informs the IDT of the medical condition of each participant, remaining alert to pertinent input from other team members, participants caregivers, as well as documenting changes in a participants medical record consistent with documentation policies.
\nJoin us in providing primary medical management in collaboration with our esteemed PACE Organization (PO) Physicians. As a Nurse Practitioner, you'll conduct assessments, develop care plans, and ensure participants receive comprehensive care while fostering a warm and supportive environment.

Collaborate with PO physicians, performing assessments and implementing care plans.

Participate in discharge planning and provide acute illness treatment.
\n - \n
Conduct home visits when necessary.
\n - \n
Assist in formulating clinical policies and procedures.
\n - \n
Participate in provider on-call rotation.
\n
Minimum Qualifications:

Master's degree in Nursing with five (5) to seven (7) years of experience required preferably in long-term care. Must possess a current, unencumbered, active license to practice in the state of PO. CDS Certification and DEA Registration required. Board Certification by ANCC or AANP.

Certification as a Nurse Practitioner with prescriptive authority required. Current BLS required.
\n - \n
A minimum of one (1) year working with the frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.

Current BLS certification.
\n - \n
Ability to travel within the Center's geographic region.
\n
