Club Service Coordinator

Provide direct care and support according to individuals’ daily living needs, including lifting, turning, positioning, transporting, bathing, dressing, grooming, oral hygiene, call-light response, activities, and restorative care Serve as shift lead by guiding and mentoring care staff, coordinating assignments, and ensuring services align with care plans and organizational standards Arrange coverage, adjust duties, or provide direct caregiving support in response to staffing gaps Communicate professionally, follow instructions, and provide customer service to individuals, families, team members, supervisors, and visitors Report changes in individuals’ physical, emotional, or cognitive condition Support clinical staff with delegated tasks, vital signs, weights, data collection, room preparation, belongings organization, admissions, and onboarding Uphold individual rights, dignity, confidentiality, and applicable policies and procedures Document care and observations accurately and confidentially in electronic medical record systems Follow safety, compliance, equipment, maintenance, emergency, and infection‑prevention protocols Assist with meal preparation, food service, eating, after‑meal care, snacks, beverages, intake documentation, and dietary changes Requirements STNA certification preferred but not required High school diploma or GED required Valid driver’s license Automobile insurance Reliable transportation Core Competencies Demonstrates expertise in providing direct care and support to individuals, ensuring adherence to care plans and organizational standards while maintaining dignity and confidentiality. Proficient in documenting care accurately and supporting clinical staff with delegated tasks.
