Hospice RN - Registered Nurse

Job Title: Registered Nurse – RN Job Location: Knoxville, TN Job Type: Full Time Position Overview The RN provides and directs care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The role includes coordinating total care plans with other healthcare professionals, ensuring continuity of patient care, conducting initial and comprehensive nursing evaluations, developing care plans within state guidelines, ensuring eligibility and medical necessity, maintaining accurate documentation, and providing high quality clinical services within the scope of practice from admission through discharge. Responsibilities Coordinate the total plan of care with other health care professionals and maintain continuity of patient care. Conduct initial and comprehensive nursing evaluation visits, determine primary focus of care, develop plans within state guidelines, and ensure patients meet home health eligibility and medical necessity. Maintain and submit accurate documentation and clinical notes describing the patient’s condition, goals, interventions, and response to home health care. Provide clinical services within the scope of practice as defined by state laws governing nursing practice. Plan and exchange information with physicians, agency personnel, patients, families, and community resources. Qualifications One (1) year of clinical RN experience. Current RN license in the state where services are rendered. Current CPR certification. Driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation to commute to multiple patient residences daily within the geographic area assigned.
