Designated Manager (245D Licensed Programs)The Designated Manager (DM) is responsible for the overall management, compliance, and operational excellence of the company's Minnesota 245D-licensed Home and Community-Based Services (HCBS) programs. This individual serves as the DHS-recognized Designated Manager, ensuring compliance with Minnesota Statutes, Chapter 245D, while fostering a culture of quality, accountability, and person-centered care.The DM will be a key leader within the organization, collaborating with executive leadership to shape operational systems, guide Designated Coordinators, maintain full regulatory readiness and drive exceptionalism with a mindset of excellence and growth.Key ResponsibilitiesServe as the Designated Manager per Minnesota Statute 245D.081, responsible for daily oversight of program operations.Ensure ongoing compliance with 245D, 245A, and related Minnesota rules, including service delivery, documentation, staff training, and incident reporting.Oversee licensing renewals, variances, audits, and investigations.Maintain and implement company policies, procedures, and corrective action plans in accordance with DHS and MDH requirements.Conduct regular site visits and internal audits to verify compliance and quality standards.Leadership & Staff DevelopmentProvide supervision, mentoring, and performance evaluation for Designated Coordinators and other program leaders.Support recruitment, onboarding, and training of staff to ensure compliance with 245D requirements.Promote a culture of respect, teamwork, and person-centered values across all service sites.Program ManagementOversee the implementation of individual service plans (ISPs) and ensure appropriate documentation of services delivered.Coordinate communication among interdisciplinary teams, guardians, and case managers.Monitor staffing ratios, scheduling, and resource allocation to maintain operational efficiency.Quality Assurance & ReportingDevelop and oversee internal quality assurance processes, including review of service delivery records, outcomes, and incidents.Submit required reports to DHS and other oversight bodies in a timely and accurate manner.Analyze data to identify trends, risks, and opportunities for improvement.Report concerns, risks, and trends to leadership.Strategic LeadershipCollaborate with company leadership to align 245D operations with broader business goals.Contribute to strategic planning, growth initiatives, and program expansion.Represent the company professionally in interactions with counties, licensors, and external stakeholders.Drive goals and proactively report progressRequirementsMust meet the Designated Coordinator qualifications under Minnesota Rule 245D.081, Subd. 1–2, including:Bachelor's degree in a human services-related field and at least one year of experience in programs providing direct support services; ORAssociate's degree (or 60 semester credits) in a related field and at least two years of experience in a licensed program; ORHigh school diploma or equivalent and at least three years of experience in programs providing direct support services.Must have a minimum of three years of supervisory level experience overseeing care or education to vulnerable populationsDemonstrated knowledge of 245D, 245A, and 245C statutes and compliance expectations.Strong communication, leadership, and organizational skills.Ability to manage multiple priorities with professionalism and confidentiality.Valid driver's license and ability to travel within Minnesota.Preferred:Prior experience as a Designated Manager in a 245D-licensed provider setting.Experience with Home and Community-Based Services (HCBS), Assisted Living, or Home Care programs.Familiarity with DHS licensing platforms (MN-ITS, PELICAN, etc.).Compensation & BenefitsCompetitive salary commensurate with experience.Comprehensive health, dental, and vision benefits.401(k) with employer match.Paid time off, holidays, and ongoing professional development opportunities.About Our CompanyWe are a growing Minnesota-based care organization dedicated to supporting individuals through high-quality home and community-based services.
