Compensation $85,000-$90,000/yearly DOE Employment Details Status: Full-Time, W2 Employee Schedule: Monday - Friday, 8 AM - 5 PM (Occasional on‑call weekends) Location: Arizona (West Valley, East Valley, and Tucson) About The Role As a Clinical Patient Care Navigator, you will have the unique opportunity to utilize your clinical background to connect with patients, families, and referral partners with our exceptional continuum of healthcare services. In this role, you will focus on strategically expanding our business within your assigned territory while strengthening existing partnerships and fostering new connections. You will play a key role in identifying and generating opportunities to grow our services for Atlas Medical, Aegis Home Health, and Aegis Hospice. Your responsibilities will include representing our organization, building meaningful relationships, and delivering impactful presentations on Home Health and Hospice care. Additionally, you will collaborate with our team to achieve monthly goals and ensure the highest standards of service and care. This position requires adaptability and a proactive growth initiative with a commitment to maintaining strong relationships and delivering exceptional care. Qualifications Hospice clinical background. 2+ years of Hospice referral relationships. Knowledge of local Senior Care Industry, hospital case management, and skilled nursing. Intuitive, solution‑oriented, and driven to achieve goals. Detail‑oriented, flexible, and self‑motivated. Excellent oral and written communication skills. Valid driver’s license (clean record) and reliable transportation. Proof of current auto insurance and car registration. Valid AZ fingerprint clearance card.
