Our comprehensive heart failure program provides APPs the opportunity to practice at the top of their license while contributing to life-saving advanced cardiac therapies./ppPosition Summary/ppSt. Francis Hospital Heart Center is seeking a highly skilled Inpatient Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Comprehensive Congestive Heart Failure Program./ppThe APP will provide advanced, evidence-based care to patients with acute and chronic heart failure across all stages of disease severity, including complex and advanced therapies. This role practices collaboratively with board-certified Heart Failure Cardiologists, cardiac surgeons, intensivists, nurses, and multidisciplinary specialists to deliver safe, effective, and compassionate inpatient care./ppProgram Overview/ppThe Center for Advanced Cardiac Therapeutics offers comprehensive management for patients with weakened heart muscles and advanced heart failure, including expertise in:/pulliAdvanced medical heart failure therapy/liliLVAD and VAD management/liliECMO support/liliCardio-oncology/liliCardiac amyloidosis and sarcoidosis/liliPulmonary hypertension/liliCardiogenic shock/liliStructural heart disease (in partnership with the Heart Valve Center)/li/ulpKey Responsibilities/pulliPerform comprehensive inpatient evaluations of patients with acute decompensated and chronic congestive heart failure./liliDiagnose and manage systolic and diastolic heart failure./liliDevelop and implement individualized treatment plans./liliManage guideline-directed medical therapy (GDMT)./liliProvide care for patients requiring advanced therapies including LVAD, VAD, and ECMO./liliAssist in the management of cardiogenic shock and complex cardiac conditions./liliMonitor laboratory results, imaging, and hemodynamic data./liliOrder and interpret diagnostic tests./liliParticipate in multidisciplinary rounds./liliAssist in advanced cardiac therapeutic interventions./liliParticipate in LVAD patient evaluation and inpatient management./liliSupport ECMO and mechanical circulatory support management (in collaboration with intensivists and cardiologists)./liliEducate patients and families regarding:/liulliDisease progression/liliMedication management/liliFluid and weight monitoring/liliSymptom recognition/liliLifestyle modifications (including plant-based, heart-healthy diet guidance)/li/ulliSupport transitions of care and discharge planning to reduce readmissions./li/ulpQuality Research/pulliParticipate in heart failure clinical trials and research initiatives./liliSupport performance improvement efforts to reduce hospital length of stay and readmission rates./liliMaintain compliance with institutional and national heart failure guidelines./li/ulpQualifications/ppEducation, Licensure Certification/pulliMasters or Doctoral degree as a Nurse Practitioner or Masters degree as a Physician Assistant from an accredited program./li/ululliCurrent NYS NP or PA license/liliNational board certification (ANCC/AANP or NCCPA)/liliDEA registration/liliACLS and BLS certification - ONLY by AHA/li/ulpExperience/pulliPreferred: 2+ years of cardiology, heart failure, ICU, or inpatient cardiology experience/liliExperience managing advanced heart failure therapies (LVAD, ECMO, cardiogenic shock) strongly preferred/liliStrong clinical decision-making skills in high-acuity settings/li/ulpCore Competencies/pulliAdvanced knowledge of heart failure pathophysiology and guideline-directed therapy/liliAbility to manage high-acuity and rapidly changing clinical situations/liliStrong interdisciplinary collaboration skills/liliCommitment to patient-centered, compassionate care/liliExcellent communication and patient education skills/liliDedication to quality improvement and evidence-based practice/li/ulpPosted Salary Range/ppUSD $105.00 - USD $105.00 /Hr./p/div
