MDS Coordinator (RN or LPN) for our Skilled Nursing Facility (SNF)

 

Compensation: $92,000 - $105,000 Annual Salary

We are looking for a dedicated MDS Coordinator for our 156 bed Skilled Nursing Facility in Baltimore, MD. This is a full-time, on-site position in the building. The MDS Coordinator owns the resident assessment process from admission through discharge, completes and transmits the MDS on schedule, and leads care plan meetings alongside the interdisciplinary team.

 

Qualifications of the MDS Coordinator:

- Active Maryland RN or LPN license

- Working knowledge of the MDS 3.0 assessment process and PDPM

- Experience in a skilled nursing or long term care setting preferred

- Strong documentation and computer skills

- Able to work full time on site in the building

 

Responsibilities of the MDS Coordinator:

- Complete, code and transmit MDS assessments within CMS timeframes

- Schedule and track the assessment calendar for all residents

- Lead care plan meetings with nursing, therapy, dietary and social services

- Review documentation for accuracy and reimbursement support

- Serve as the building resource for resident assessment questions
