RN Case Manager Pay: $37.44–$45.00/hour, based on experience Job Type: Part-time (up to 20 hours/week) Location: Fairmont, MN (In person) Schedule: Flexible, part-time schedule up to 20 hours/week, coordinated around your availability and patient needs About the Role Baywood Home Care is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate care, support patients and families, and help individuals maintain their independence in the comfort of their own homes.
