RN Skilled Nursing

The Registered Nurse (RN) provides skilled nursing care in a Long-Term Care/Skilled Nursing Facility setting. The RN utilizes the nursing process and critical thinking to assess, plan, intervene, and evaluate patient care in collaboration with an interdisciplinary team. Responsibilities include medication administration, patient education, documentation, and participating in performance improvement activities. The RN supports patient safety, manages complex situations, and promotes professional development and shared governance. A current Illinois RN license and CPR/BLS certification are required, along with graduation from an accredited nursing program and at least 2 years of relevant experience. Responsibilities:

• Perform patient assessments, develop and execute individualized care plans.
• Administer prescribed medications and treatments according to nursing standards and state laws.
• Document patient care accurately and comprehensively following system and entity standards.
• Collaborate with interdisciplinary team members to provide comprehensive patient care.
• Provide patient and family education tailored to their understanding and needs.
• Participate in department/unit performance improvement and shared governance activities.
• Provide clinical expertise and guidance to other units/departments as needed.
• Identify and resolve system breakdowns, escalating issues through appropriate channels.
• Manage complex patient care situations and delegate tasks appropriately.
• Engage in personal development and support continuous learning and evidence-based practice.
• Participate in peer review and feedback processes.
• Maintain timely patient flow and respond to telephone triage when applicable.

• Current Registered Nurse license in Illinois.
• CPR/BLS certification.
• Graduation from an accredited nursing program with education verification.
• Minimum of 2 years nursing experience in hospital, home health, or hospice care.
• Clinical assessment, medication administration, and treatment planning skills.
• Strong interdisciplinary team collaboration abilities.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced healthcare environment.

• Experience in Long-Term Care or Skilled Nursing Facility settings.
• Knowledge of evidence-based practice guidelines and quality improvement processes.
• Experience with telephone triage and ambulatory care nursing.

• Work involves collaboration with interdisciplinary teams in a Long-Term Care environment.
• Must be able to manage complex patient situations and escalate issues appropriately.
• Participation in shared governance and peer review activities is expected.

• 36 hours per week scheduled during day and evening shifts.
• Shift assignments may vary based on department needs.
