RN Case ManagerLocation: Hybrid, driving in the field. Position Type: Full Time Salaried. Schedule Requirements: Primarily day shift, but also includes a required on-call, weekend, and holiday rotation. Sign-On Bonus: This position is BONUS Eligible at 6 months of successful employment. Rainbow Community Care is highly regarded for excellence in hospice, palliative, and guided care and invites you to join in our mission of enhancing lives throughout aging, illness, and loss.Purpose of the Position Due to exponential growth, Rainbow Community Care is seeking a dedicated Registered Nurse Case Manager for home hospice.Key ResponsibilitiesDevelops and supervises the planning, coordination, and delivery of patient/family care.Implements physician orders and maintains communication with patient's primary care physician.Educates family members on patient care skills, meds, treatments, use of equipment, and signs and symptoms of the dying and grieving process.Maintains communication with interdisciplinary team members through participation in bi-weekly meetings.Visually observes and interprets changes in behavior and psychological and physiological status of patient for ongoing assessment.Develops nursing diagnosis and individualized care plans for each patient and modifies care plan based on ongoing assessment of patient condition.Documents medication given, care given, patient condition, assessment, and patient diagnosis on patient chart.Completes documentation in a timely manner and maintains integrity of the patient chart.Attends trainings and staff meetings as required.QualificationsPersonal Qualifications include:Ability to handle stressful and emergency situations.Ability to adequately apply decision-making and problem-solving techniques in resolving issues.Ability to drive to and from care facilities, patient homes, and other locations as necessary.Ability to maintain healthy boundaries and positive interpersonal relationships.Ability to be self-directed and maintain professional representation of Rainbow Community Care at all times.Professional Qualifications include:Clinical management experience preferred.Hospice, Palliative Care, Home Health, ER, Oncology and/or Med Surg experience preferred.Wisconsin RN licensure is required and must be maintained.Valid Wisconsin Driver's License is required and must be maintained.Current auto insurance is required and must be maintained.BenefitsFlexible, fun, and caring work environment401k Retirement Plan with up to 4% Company MatchHealth & Dental InsuranceVision InsuranceFlexible Spending & Health Savings Accounts100% Company-Paid Group Life Insurance100% Company-Paid Short-Term & Long-Term Disability InsuranceGenerous Paid Time Off & Paid HolidaysMileage ReimbursementTuition ReimbursementPet InsuranceAbout Rainbow Community Care Rainbow Community Care provides services that support patients, families, and community members throughout the journey of aging, illness, and loss.
