Job Role: Physical Therapist (PT) – Mobile Outpatient Care

Location: Palm Beach County, FL
Job Type: Full-Time
Service Area: Palm Beach Gardens to Boynton Beach
Compensation: $70–$75 per regular visit | $75–$80 per evaluation

About the Opportunity

We are seeking a compassionate and clinically skilled Physical Therapist (PT) to join a growing concierge-style therapy practice serving patients throughout Palm Beach County.

This is a mobile Medicare Part B position providing outpatient-level physical therapy directly in patients’ homes. The practice is built around a patient-first model, with approximately 53–60 minutes of dedicated, one-on-one treatment time per visit .

Unlike a traditional high-volume therapy environment, this position allows the Physical Therapist to focus on one patient at a time, develop meaningful relationships, and provide individualized, high-quality care.

The ideal candidate will have excellent clinical judgment, outstanding communication and interpersonal skills, strong follow-through, and the ability to work independently while traveling throughout the assigned Palm Beach County territory.

Responsibilities

• Perform comprehensive physical therapy evaluations and assessments
• Develop individualized plans of care based on each patient's condition, functional limitations, and goals
• Provide one-on-one physical therapy in patients’ homes
• Treat a variety of orthopedic and neurological conditions
• Provide therapeutic exercise, manual therapy, balance and gait training, functional mobility training, and other evidence-based interventions
• Work with patients recovering from orthopedic injuries, surgeries, neurological conditions, balance deficits, and mobility limitations
• Monitor patient progress and modify treatment plans as appropriate
• Educate patients and caregivers regarding home exercise programs, safety, mobility, and self-management
• Complete accurate and timely clinical documentation
• Communicate professionally with patients, caregivers, and other healthcare professionals
• Maintain excellent follow-through and continuity of patient care
• Travel between patient locations throughout the assigned service area

Qualifications

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
• Current and unrestricted Florida Physical Therapist license
• BLS certification
• Valid driver’s license and reliable transportation
• Strong clinical assessment, evaluation, and treatment-planning skills
• Ability to work independently in a mobile patient-care environment
• Comfortable treating orthopedic and neurological patients
• Excellent verbal and written communication skills
• Exceptional interpersonal and patient relationship skills
• Strong organization, reliability, and follow-through
• Willingness to travel throughout Palm Beach County, primarily from Palm Beach Gardens to Boynton Beach

Experience in one or more of the following areas is preferred but not required:

• Outpatient physical therapy
• Orthopedic rehabilitation
• Neurological rehabilitation
• Geriatric physical therapy
• Balance and gait rehabilitation
• Post-operative rehabilitation
• Home-based or mobile therapy
• Home health
• Medicare Part B

Compensation

• $70–$75 per regular patient visit
