Overview Social Worker Supervisor. The Center for Elders’ Independence is a PACE organization that uses an interdisciplinary team approach to care planning and implementation to provide high‑quality, affordable, integrated health‑care services to the elderly. This includes an Adult Day Health Center and aims to promote autonomy, quality of life, and community living for participants. Under the supervision of the Center Director, the Social Worker Supervisor is a mental health professional who helps frail and elderly PACE participants find solutions to a wide range of social problems. The supervisor serves as an integral member of the Interdisciplinary Team (IDT), performs psychosocial assessments, develops and implements plans of care, and facilitates communication among participants, families, and CEI staff. The position also serves as the location contact for the social work discipline and supervises the location Social Workers and Social Work Assistants. The salary range is $92,150–$138,225 per year, with an hourly rate based on market comparability and individual experience, skills, abilities, and work history. Responsibilities Act as the point person for the social work team at the assigned center. Participate in interview and hiring processes with other leaders as directed. Provide orientation and training to new social workers and assistants in conjunction with the Social Work Functional Manager. Collaborate with the Social Work Functional Manager across centers to standardize best practices and protocols in the department. Represent the department in quality and process‑improvement initiatives and meetings. Review and approve direct‑report time cards for accuracy and completeness. Ensure adequate coverage of the social work team during vacations and time‑off requests, and approve or deny requests based on business needs. Maintain current assessments for the team. Complete performance evaluations and participate in employee development and coaching. Organize psychosocial rounds with the Center team, psychiatrists, and/or CEI LCSW team. Support standardization of best practices, supervision, and implementation of behavioral health training models and internal participant referrals. Coordinate student/intern placements with corresponding schools and universities as needed. Refer participants with severe mental challenges to behavioral health resources. Develop and implement comprehensive, integrated care plans as an IDT member, monitor progress, evaluate behavioral adjustments, and communicate changes to team members. Conduct initial and reassessments, and update care plans while evaluating participants’ social service needs and family circumstances. Make referrals to community resources for financial management, benefits eligibility, and other participant needs. Manage assigned caseload, prepare narratives and reports, and document case files. Respond to and investigate complaints of elder abuse. Conduct family and/or caregiver conferences as needed. Develop and maintain relationships with agencies such as the Department of Social Services, psychiatric facilities, hospitals, social agencies, and participate in discharge planning. Maintain confidentiality of all company procedures, results, and participant information per HIPAA principles. Make home visits, hospital visits, and travel to meet participants and service providers. Perform related duties as assigned. Qualifications Master’s degree in Social Work from an accredited school; experience with medically frail elderly is desirable. 1–2 years supervisory experience required. Demonstrated ability to work in a multicultural and multidisciplinary team. Ability to work with minimal supervision. Excellent oral, verbal communication, and facilitation skills. Experience with severe mental illness is desirable. Bilingual ability preferred (Spanish, Korean, Cantonese, Mandarin). Valid California driver’s license, acceptable driving record, insurance, and reliable transportation. Current CPR and First Aid certification.
