Certified Home Health Aide - CaregiverAs a Certified Home Health Aide (HHA), you will provide care to clients in their home, following their plan of care. You make a positive difference each and every day by demonstrating care and compassion to our patients.This position has a designated home base facility and may require occasional travel to additional locations. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided.Responsibilities:Patient Care & Service - Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands.Communication - Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly.Documentation - Accurately and completely document client care on activity sheet.Compliance - Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements.Minimum Qualifications:Active Home Health Aide (HHA) certificationHHA with a lapse of more than two years of employment (must complete written exam and 1-day HHA Skills Competency Class within 30 days of hire)Certified Nursing Assistant (CNA) willing to complete a written exam and 1-day HHA Skills Competency Class within 30 days of hireOut-of-state HHA or Nurse Aide training with documented competency evaluationNursing student (Fundamentals of Nursing completed within the past 24 months)U.S. Military-trained medic or medical technicianPreferred Qualifications:High school diploma or equivalent1 year experience working with the frail elderly populationCompassionate, patient focused and dependable professionalCritical thinking skills, decisive judgment and the ability to work with minimal supervisionExceptional documentation and communication skillsAbility to follow verbal and written instructionsEducation:LICENSES / CERTIFICATIONS:NYS DL - valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV)Physical Requirements:M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.Any physical requirements reported by a prospective employee and/or employee's physician or delegate will be considered for accommodations.Pay Range:$18.65 - $19.75City: RochesterPostal Code: 14609The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
