This role offers the opportunity to build meaningful relationships with patients, work independently, and make a tremendous impact in the community—while receiving competitive pay and excellent benefits.Job SummaryThe Hospice Admission Registered Nurse (RN) serves as the first clinical point of contact for new hospice patients and families. This role conducts admission consultations and comprehensive assessments to determine hospice eligibility and develop individualized plans of care. The Admission RN delivers compassionate hospice nursing care in patients’ homes and community settings while collaborating closely with the interdisciplinary team.Key ResponsibilitiesConduct comprehensive hospice admission assessments for patients and familiesPerform physical assessments and obtain detailed medical historiesDetermine hospice eligibility using assessment data and clinical judgmentDevelop individualized plans of care based on nursing diagnoses and patient goalsImplement palliative and preventive nursing interventionsAdminister medications and treatments per physician ordersEducate and counsel patients and families on care needs and comfort measuresAssist with activities of daily living (ADLs) while promoting comfort and independenceCommunicate patient care needs with the hospice interdisciplinary teamMaintain accurate and timely clinical documentationQualificationsCurrent Registered Nurse (RN) license in California e12aMinimum of two (2) years nursing experience; hospice, home health, or public health preferredCurrent CPR certificationStrong assessment, communication, and time‑management skillsAbility to work independently with minimal supervisionThe employer for this position is stated in the job posting.
