The Case Manager, Mental Health Professional (MHP) provides master’s-level clinical services within CHMHA’s licensed residential programs to meet Washington State residential staffing requirements for clinical oversight and direct therapeutic services. This position delivers direct mental health treatment to residents, supports treatment planning and documentation, and contributes clinical expertise to the on‑site care team. This role is not a supervisory or leadership position, but functions as a clinical resource to support non‑MHP case managers and residential staff by providing consultation, guidance on clinical interventions, and continuity of care within a multidisciplinary team model. The MHP ensures residents receive appropriate therapeutic support while supporting compliance with licensing, contract, and quality standards. SCHEDULE
Monday‑Friday, 8:00am‑4:00pm (Full‑Time, 40 hours/week) RESPONSIBILITIES Provide direct individual mental health counseling and therapeutic interventions to residents in accordance with service plans and clinical need. Provide clinical consultation and guidance to residential staff and nonMHP case managers through collaboration and shared clinical planning, as needed to support resident care; this role does not include formal supervisory authority. Contribute to meeting King County residential licensing requirements by providing required clinical services and therapeutic support to residents, consistent with Mental Health Professional (MHP) standards. Serve as Primary Case Manager for assigned caseload, including advocating for and coordinating resident needs and services; ensuring Service Plans, Crisis Plans, Continuing Stay Reviews, and other documentation are completed on time and kept up to date; and providing consistent contacts and skills training to work towards goals. Support the clinical work of nonMHP case managers through collaboration, shared documentation standards, and consultation on treatment planning, while maintaining a non‑supervisory role. Coordinate with relevant internal and external departments to coordinate resident referrals, ensure timely intake into program, and review of individualized plans. Collaborate with care team to ensure all residents access needed treatment, support, case management, and appointments, including transportation to external providers. Ensure all interactions represent the five principles of trauma‑informed care, including safety, choice, trustworthiness, collaboration, and empowerment. Facilitate, monitor, and support medications and medication windows to ensure the seven rights of medication use are followed: right person, right medication, right time, right dose, right route, right reason (including refusal), and right documentation according to agency provided medication training and within assigned role responsibilities. Actively participate in Shift Change meetings and complete associated documentation. Complete daily documentation in electronic medical record on resident interactions, medications dispensed when assigned, and other required documentation, ensuring an interaction or observation and subsequent documentation are completed for all residents across the team. Practice good financial stewardship of agency and contract resources, including how time is being spent, supplies are used, property is being maintained, etc. Meet identified key performance indicators and metrics for the position as identified for the program, division, and agency. Participate in discharge planning and written summaries of treatment as needed for client referral and releases of information. Assist with and support on‑site cleaning, including skills training with residents to maintain personal and community spaces, laundry, and preparation for regular pest treatments. Provide immediate crisis intervention and resolution as needed. Always maintain resident and staff confidentiality. Completing assigned work related to maintaining and adhering to licensing responsibilities, such as Washington Department of Social and Health Services (DSHS) and Department of Health (DOH), as assigned by supervisor. Providing skills training and assistance to residents in Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) per their service plan and individual needs, such as assistance in hygiene needs, feeding, personal hygiene, cleaning, finances, transportation, etc. Includes preparing resident meals when cook is not on duty and per menu. Learning and executing skills and techniques promoted in the State of Washington mandated training, Fundamentals of Caregiving. Coordinating care, such as scheduling, attending, and/or arranging transportation for appointments and other case management needs. Maintaining, and documenting in, resident charts, activity logs, meal charts, etc. following agency, county, state, and federal guidelines. Facilitating regular group and individual activities, encounters, and specialized resident events, such as outings and holiday gatherings. Assisting in admission and discharge planning per supervisor direction. Participating in fire drills, resident orientation, chores, and resident room maintenance during resident move‑in and move‑out. Monitoring and managing facility milieu for visitors, television time, and quiet times. Attending all scheduled meetings and supervisions. Staying awake on all shifts. Performing other duties as assigned. QUALIFICATIONS Master’s degree in a qualifying behavioral health or social science field and ability to meet Washington State requirements for designation as a Mental Health Professional (MHP). Experience serving adults with severe and persistent mental illness. Apply for Agency Affiliated Counselor credential with Washington Department of Health within 30 days of hire, if not otherwise licensed, and obtain and maintain registration and/or licensure. Successfully pass criminal background check before hire date. Able to complete all Assisted Living Facility Training requirements per DSHS: Complete fingerprinting requirements within 14 days of hire. Obtain/maintain current Food Workers Card within 14 days of hire. Obtain/maintain valid First Aid/CPR card or certificate within 30 days of hire. Complete 75‑Hour Long‑Term Care Worker Basic Training plus Mental Health and Substance Use Disorder specialty training within 120 days of hire. Obtain Home Care Aide certification within 200 days of hire. Complete 12 hours of continuing education annually. Complete Tuberculosis (TB) test within first 2 weeks of hire with acceptable results. Hold a valid license for 1 year with clean driving record to use agency vehicles. Have legible handwriting and ability to do simple mathematics. Able to navigate and comprehend charts in electronic medical record. Able to work independently and within a team. Able to communicate information and ideas so others will understand. Able to exchange accurate information in these situations.
